Q: How do I enroll in the program?
A:
Simply fill out the one page participation agreement form. Click
on the link below to download the form in Microsoft Word format. You may
also obtain
one by contacting the GPCF office. Please mail or fax it back upon
completion.

Q: When will I receive my first client referral and who sets up the appointment?
A:
A GPCF Client Representative will call your office and request to speak with the
"contact person" you have selected to act as the liaison between our
office and you. At this time, we can address any questions you may have about
how the client referral process works. GPCF clients will be given your office
number at the time of the referral. It will be the client’s responsibility to
call for your first available appointment.
Q: How can I be assured that I will not get more than my fair share of
nonpaying clients?
A:
Some providers, by virtue of specialty or location, might see more clients
than others in a year. However, each time a your name is given out as
a referral, your name will be rolled to the bottom of the referral list.
GPCF only gives 1 referral every 30 days to a provider unless you specify that
you will see more clients on the participation agreement form.
Q: If I see a GPCF client at no charge for the initial visit under the
guidelines of the program, am I then able to bill that client for subsequent
visits?
A:
Your
commitment in accepting a client through the referral system is to see the
client for one free or reduced rate visit. Arrangements for subsequent visits
are between you and the client.
Q: What happens if the client needs prescription medications?
A:
Through the
cooperation of pharmaceutical manufacturers, various
approved products are made available to GPCF clients free of charge with an
"active" scrip card. Clients must call the client toll-free line to
receive a referral to a participating pharmacy. Most pharmacies have
agreed to waive their dispensing fees on donated products. Prescriptions
can be filled for up to a 100-day supply of a donated medication if written as
such by the physician. If the medication
is not donated to GPCF, the client is responsible for payment of the
medication. A current approved product list can

Q: If I see a client who needs to be referred for specialty care, diagnostic
tests and/or surgery, how is this handled?
A:
Limited
services are available for specialty care, diagnostic tests, and/or
surgery. Based on your preference, you may have the client contact GPCF to see
if that service is available through the program. Many providers opt to refer
clients through their own network of physicians. This is acceptable as long as
the specialist to whom you are referring agrees to see the client for the
initial office visit at no charge or at a reduced rate.
Q: How can I be certain that the client coming to my office is enrolled with
GPCF? Am I responsible for seeing family members without a referral?
A:
We will fax a GPCF referral form to your office on any clients we refer to
you. All clients are also advised to be prepared to present proof of eligibility.
You may call the provider
toll-free line to verify a client’s eligibility status. Under no
circumstance are you responsible for seeing clients without a referral or a
scheduled appointment.
