The purpose of the screening tool is to be a guide for identification and referral to Georgia Cares. This was created with state partners in an effort to streamline existing processess as it applies to providing services to victims of Domestic Minor Sex Trafficking in Georgia.

Remember, the indicators of trafficking and exploitation delineated in the tool are meant to serve as a guide for determining whether there is sufficent evidence to refer a youth to Georgia Cares.

REFER A YOUTH TO GEORGIA CARES

Click on the image to complete a Georgia Cares Referral and Release of Inforamtion electronically. 
 
Important Note: Georgia Cares requires a Georgia Cares Release of Information/Consent Form signed by the legal guardian for each youth that we serve. This form is needed prior to Georgia Cares engaging in services with the youth. The Release of Information/Consent Form allows us to work with and access records from the youth, their family, and other applicable partners. This form should be submitted when submitting the referral.

If you are unable to get the release of information signed at the time of referral, please refer to these FORMS. You may complete these seperately, but please note that services will not be provided until the signed release of information is received by Georgia Cares.
